Immunizations

Immunization Requirements

Don't miss out on registering for good classes!

Submit your Immunization Record now to avoid a hold on your registration.

The Code of Virginia (Section 23-7.5) requires all full time students to provide documentation of their immunizations.

The VCU Certificate of Immunization must be completed; it is also available from University Student Health Services (USHS) offices.

Proof of the following is required:

  • 2 measles
  • 2 mumps
  • 2 rubella
  • 1 tetanus (within the past 10 years)
  • 1 meningococcal meningitis or signed waiver
  • 3 hepatitis B or signed waiver
  • 1 tuberculosis screening (also required for part-time students born outside the United States)

Vaccines are available for a fee at USHS for incoming students. However, vaccine administration prior to arriving on campus is encouraged, as USHS does not bill health insurance.

If your specific program requires you to submit a Certificate of Immunization to the program office, be sure that you also send a copy to University Student Health Services.

Students attending New Student Orientation may hand-deliver the Certificate of Immunization to the Academic Campus USHS Clinic, which is located in the Sports Medicine Building, 1300 W. Broad St., Suite 2200.

Religious Exemption

Any student who objects on the grounds that administration of immunizing agents conflicts with his religious tenets or practices shall be exempt from the immunization requirements unless an emergency or epidemic of disease has been declared by the Board of Health. An affidavit of religious exemption must be submitted on a Certificate of Religious Exemption (Form CRE-1) which may be obtained at any local health department, school division superintendent's office or local department of social services. This form must include a notary public's seal.
